Individual and Family Plans

Need the legal name, address, social security number and date of birth for you and your:

  • Spouse or unmarried partner
  • Children who live with you (even if they file a tax return)
  • Dependents (anyone you claim even if they don't live with you—as well as anyone who's under 21 who does live with you)

Have them handy when you apply so we can make sure they're covered under your plan.

Dust off last year's federal tax return so you can report your:

  • Marriage Tax Status File separately or jointly?
  • Dependents: Who do you claim on your return?

This includes you and everyone in your household. Things that would be good to have on hand are:

  • W-2 Forms & Pay Stubs: Includes wages, salaries and tips
  • Household Net Income: Include any self-employment or self-owned businesses
  • Unemployment: Any compensation you received in 2025
  • Social Security Payments: Includes disability payments
  • Alimony: Only for divorces and separations finalized by 1/1/2025
  • Retirement: Includes pension income and most IRA or 401K withdrawals
  • Investments, Rental and Other Taxable Income: Includes dividends, interest, rent payments made to you, etc.

If you and/or your household is covered now, make sure you collect everyone's policy numbers. (Tip: You can find that info on their insurance cards.)

If someone is helping you with your application, you'll be asked to enter their information. Could be an agent, broker, application counselor, or another health insurance professional.
